Whether it’s a group outing, a recreational activity, or a shared celebration, we help people feel seen, valued, and connected.

Our Services

Our Commitment to Person-Centered Care

Journey Found provides compassionate 24‑hour support and assistance with all aspects of daily living, with the goal of creating warm, comfortable, and truly personalized home environments for every individual we serve.

We partner closely with each person—and their support network—to build a customized care plan that nurtures overall wellbeing. This approach goes far beyond physical health. It emphasizes meaningful relationships, emotional connection, stability, and a sense of belonging. We understand that “home” is not just where someone lives, but where they feel valued, supported, and understood.

Our services include regular community outings, trips to the shore, visits to favorite stores, coffee outings, concerts, and wellness-based activities. These experiences help residents stay connected to the broader community and enjoy the things they love. Within the home, our staff provides individualized support, ensuring everyone feels engaged, respected, and empowered in their daily lives.

Quality of life is central to every care plan we create. By fostering both independence and connection, we help individuals with disabilities live full, meaningful, and community‑integrated lives.
