About Journey Found
In February of 2013, 51 people with intellectual disabilities, their families, and the 192 dedicated staff members who supported them, had their lives disrupted when they were notified that the agency providing their homes and other vital services was no longer able to serve them as of June 30, 2013. In a heroic effort, the parents of the people served came forward and committed to creating an agency for the sole purpose of the continuity of care for their loved ones and the other people supported.
Journey Found was born from the love and commitment of these parents and the dedication of the staff who served them. On May 16, 2013, Journey Found was incorporated and the door opened on July 1, 2013, to a fully formed nonprofit organization with no disruption for those enrolled in the programs. This odyssey not only preserved services for those 51 individuals and jobs for the 192 staff, but also began a much-needed process of culture change and values clarification. The organization went from a top down, compliance-focused structure, to a vision-driven agency committed to quality and governed by families.

Journey Found provides 24 hour support and needed care in all aspects of daily life with the goal of creating warm and comfortable spaces for everyone.
We work together to create a personalized care plan that supports wellbeing for the people we support that goes beyond just physical health. We forge life-long relationships, and understand that home is more than just a place to live.
Our services include regular outings into the local community, trips to the shore and favorite stores, coffee runs, concerts, and wellness activities to help our residents maintain their connections to a wider community. Within the home, our staff focuses on providing individual support to engage and connect with each person in our care. Quality of life is central to our personalized care plans, and connection to the community is key to people with disabilities living a full life.
Our Approach:
Journey Found uses a specific and tailored team approach to develop personalized plans for each person supported.
Our team includes Clinical Behavioral Specialists (BCBAs), Registered Nurses (RNs), highly trained program directors, and program managers. Each member of the team works together with families and guardians to build a personalized support plan, ensuring all aspects of care are addressed.
Available Support:
Each individual we work with has their own needs. Our team will create a plan that gives each person the support, social interaction, and independence they want, while offering rest and respite for family and caregivers.
