Services

We want the people we serve to experience personal growth and to be as independent as possible. Journey Found uses a team approach to develop personalized plans for each individual. Our Clinical Behavioral Specialists (BCBAs), Registered Nurses (RNs), highly trained Program Directors and Program Managers work alongside families and guardians to ensure all behavioral aspects of care are addressed.

24/7 Staffed Homes 

Journey Found provides a variety of living arrangements across North Central, and southeastern Connecticut for adults with Autism, intellectual disabilities, and other co-occurring challenges.

Person-centered caring is evident in the small group homes of three to four individuals. In addition to group homes, people are also supported in apartments, either on their own or with 1-2 housemates.

Activities
Staff members assist the people we support with activities of daily living, which may include but is not limited to: personal hygiene/self-care tasks, dressing, cleaning, cooking, and budgeting. Our staff is dedicated to providing a positive, therapeutic environment where we encourage participation in all activities while remaining helpful, supportive, and respectful of differences with regards to unique needs, age, gender and culture of the person being supported.

Health
Health maintenance, medication administration and health care coordination by agency RNs. This includes assisting people to participate in any health-related activities as medically recommended and in accordance with state regulations communicating all health needs with guardians.

Skill Building
Journey Found promotes skill building programs, both formal and informal, as a means of helping people learn new skills and increase their independence. Community involvement is also an important part of our daily living. Staff provide transportation for people to appointments, activities, jobs, grocery shopping and outings including dining out and visits to favorite local spots.

Home-Based Supports and In-home Services

Journey Found supports adults in their own home, whether they are living with their family, alone or others. Support may range from around the clock care or just a few hours a week.

People supported at home may receive support in all aspects of their daily lives, such as accessing the community for shopping, recreation, and services, providing enrichment activities, assistance in taking medications, or companionship. We may also provide Clinical Behavior Support Services and Health Care Coordination.

Community-Based Supports

Staff may work to support adults in individualized pre-vocational, vocational, recreational, and/or retirement enrichment activities. Support is offered up to 30 hours per week and may be based out of the person’s home, in the community, or at the Journey Found office. These supports are provided 1:1 or in a small group, with no more than four people with one staff person.

Behavioral Services

Our professional team includes clinicians, nurses, and program staff who ensure  a person-centered approach is always used for each person we support. Journey Found uses Positive Behavioral Supports utilizing ABA principles that are strength based and person centered. Our team works closely with all of the person’s health providers to ensure a comprehensive team approach is maintained at an optimal holistic level. At Journey Found, our Behavioral Health team is available 24 hours a day, 7 day a week.

Financial Services

Managing care for a loved one can be a complicated pathway. At Journey Found, we provide assistance to help guardians and people we serve to best manage their benefits and accessibility to funds.

Services our Financial Department provides include:

  • Completing DSS applications for all people new to Journey Found, making sure the necessary Cash Assistance, SNAP or Husky benefits are in place.
  • We process redetermination for benefits for the individuals we support.  This includes sending in the required documentation requested by DSS (Bank Statements, Paystubs, Funeral Accts., etc.)
  • Preparing all individual’s financial records for licensing purposes.  This includes monthly cash on hand records, PEX/Bank statements and all copies of cashed checks and deposits.
  • Setting up all new houses/individuals with their necessary utilities.  (Gas, Water, etc.)
  • Processing and paying all check requests for the  individuals we support.
  • We manage and maintain all financial records, applications and individual personal information for the people we serve.
